Eastern Plumas Health Care reports that its California Advancing and Innovating Medi-Cal — or CalAIM — department has moved offices in Loyalton. Previously located at the old barber shop, down the hall from Hair Faire, the new office location is in the old dental office, down the alley from the Loyalton Medical Clinic. The address is still the same: 703 Main St.
EPHC’s CalAIM team currently has 115 members and serves adults and children covered by Partnership HealthPlan of California throughout Plumas and Sierra counties. Providing assistance for individuals from vulnerable populations to access available resources, the team members meet community members where they live, said EPHC. They help decide what is important to their members as individuals and work to carve out goals that assist them with meeting desired outcomes, with a focus on improving their ability to manage their own health care.
The CalAIM office is available by calling 530-832-6622 or emailing [email protected]. The team does not currently have set office hours, but clients can call ahead to schedule a meeting. Staff are typically available in person between 9 a.m. and 4 p.m. Monday through Thursday.
